you need to go to the local Citizen Registration Office with your passport, the birth certificate of your child and a current photography.
You need to make an appointment and tell them that you also need a translator in your language to avoid problems.
If you have any further questions, don't hesitate to ask.

Thank you very much for your response. My child is in India. I would like to apply for german passport at the Chennai German consulate, India. is the Indian adoption recognized by German law? Do I need to apply for the adoption to be formally recognised by the competent court in Germany under the German "Act on the Effects of the Adoption" "*Anerkennungsverfahren nach dem Adoptionswirkungsgesetz -**(AdWirkG)"? Please let me know. Thanks alot !!!

you need the birth certificate anyway, translated in German for the application. It is formally recognized but you need to apply for this and have all documents translated in German. If you have the German citizenship and your child is going to live in Germany as well, there should be no problem in the application process.
